Also, please confirm that you have the proper authorization for your visit and any procedures that may be performed.

Please present your insurance card at the time of your visit to help us determine your co-pay and office visit, laboratory and hospital coverage. Advanced Women’s Health Specialists’ friendly office staff is happy to assist you and will submit your insurance claim form.

Payment is due when services are rendered. We accept cash, checks and credit cards.
